City claimed their third title under the Catalan coach, playing some scintillating stuff along the way.
The Etihad outfit lost just six games over the campaign, scoring 83 times and conceding just 32.
Guardiola joins Ruben Dias as City winners this weekend, with the central defender earlier being named as Player of the Season.
The former Barcelona man beats Marcelo Bielsa, David Moyes and Brendan Rodgers to the award.
